This model reflects the thinking about a new model of support for people with a physical or sensory impairment emerging from work being undertaken by Cheshire Social Services and partners. It explores the impact on, and potential relationships between outputs for service users in terms of their extent of impairment and their extent of disability as well as the impact this has on the potential uptake of services.
The model remains experimental and will need to evolve as people refine the new model of care being espoused.
The enabling elements of the 'living options' framework are identified opposite along with issues that will impact on levels of impairment. Outputs reflect the impact on impairment and disability and the possible impact on service uptake.
The disablement indicator reflects the extent to which 'living options' are absent. Each theme within the living options model is assessed on a scale of 0 to 10, the better the score the nearer the disablement indicator moves toward 0.
The impairment indicator is a reflection of early recognition of a long term condition and also the effectiveness of rehabilition. Any delay or sub-optimal rehab contributes to a score with the goal again being to reduce this impairment indicator to as close to 0 as possible.
The model consists of a number of steps:
- A model run that indicates outputs for the baseline position.
- Step 2 - allows impairment factors to be explored.
- Step 3 - allows the 'living options' to be explored.
- Step 4 - allows all experimentation with all the inputs.
